Delivery Schedule



In house turnaround times vary depending on the specifics of each job, including stock availability, technician scheduling, and project complexity. If you have a rush order or a particularly complex fabrication, please call us to confirm timing and technician availability. Please note the day of arrival does not count towards fabrication time.



New Customers



All new customers are required to complete a credit application form and speak to a representative from our office to establish an account with Grubbs O&P Labs. The credit application form can be found in the "Forms" section of this website. Please download, complete and return this form via email or fax at your earliest possible convenience.

Please note that fabrication will not begin until the completed application is received. Until credit terms are approved, all new customers will be billed by credit card unless other arrangements have been made in advance.



Billing Policy



Orders must be accompanied with a work order. Appropriate forms can be downloaded under the "Forms" portion of this website for your convenience.

  1. Grubbs O&P Labs has a net 30-day billing cycle. Any customer with open invoices over 30 days are subject to credit card prepayments on all fabrications until the full invoices over 30 days have been paid.
  2. Any invoices past due over 30 days are subject to a 20% late fee of the total amount past due.
  3. Grubbs O&P Labs pricing and policies are subject to change without notice.
  4. All collection costs and/or attorney's fees that are incurred by Grubbs O&P Labs in an effort to collect payment shall be paid by the company in default with Grubbs O&P Labs.
  5. By submitting fabrications to Grubbs Orthotic & Prosthetic Labs, you hereby agree to these policies.


Disclaimer



All Prosthetics and Orthotic devices fabricated by Grubbs O&P are done so in accordance with measurements and instructions provided by the practitioner. Grubbs O&P has no knowledge of the physical state or diagnosis of any patients for whom these devices are manufactured. The O&P company and the practitioner are solely responsible for these factors. Please inspect all items upon receipt. All errors, defects or damages must be reported within seven days. Grubbs O&P guarantees quality of all goods fabricated by our lab. In the event you are not satisfied, please contact Grubbs O&P so that we may rectify the problem.
